Face Covering Required

Dear Patients and Families,

Due to the continued increase in the amount of COVID-19 cases in our area, our practice has decided to implement a new policy regarding face coverings.

Effective immediately, face coverings are REQUIRED in order to enter either of our locations. This requirement is for anyone entering our offices over the age of 2. We ask that you bring your own face covering whether it be a mask, scarf or a bandanna as it will need to be securely in place prior to entering the practice.

We strive to ensure that our staff, patients and their families stay as healthy and safe as possible. We also want to protect the small children who are unable to wear a mask. If you have any questions regarding if your face covering is appropriate, please call our office or visit

Thank you for your understanding and cooperation!
