Well Child Care

At Wake Forest Pediatric Associates, we believe that routine well-care visits are vital to the health of your child. These well-child visits focus on many aspects of your child’s growth and development. Please click on the links below for details about each visit.

Newborn

We recommend an initial newborn visit in our office at 1-2 days after discharge from the hospital (as recommended by the doctor).

At this visit you can expect the provider to examine your baby and discuss the following: growth and development, safety, nutrition, sleep, and newborn care.

We will weigh your baby without the diaper or clothing.

Your child may receive the Hepatitis B vaccine if it was not given in the hospital.

You will be given the opportunity to discuss your concerns and ask any questions that you may have.

2 Weeks

We recommend your child be seen in the office at 2 weeks of age.

At this visit you can expect the provider to examine your baby and discuss the following: growth and development, safety, nutrition, and sleep.

We will obtain a naked weight, length, and head circumference.

Your child may receive the Hepatitis B vaccine if it was not given previously.

You will be given the opportunity to discuss your concerns and ask any questions that you may have.

2 Months

We recommend your child be seen in the office at 2 months of age for a well-child visit.

At this visit you can expect the provider to examine your baby and discuss the following: growth and development, safety, nutrition, and sleep.

We will obtain a naked weight, length, and head circumference.

Your child will receive the following vaccines: Pediarix (combination of DTaP, Hepatitis B, IPV), PedvaxHIB, Prevnar, and RotaTeq.

You will be given the opportunity to discuss your concerns and ask any questions that you may have.

4 Months

We recommend your child be seen in the office at 4 months of age for a well-child visit.

At this visit you can expect the provider to examine your baby and discuss the following: growth and development, safety, nutrition, and sleep.

We will obtain a naked weight, length, and head circumference.

Your child will receive the following vaccines:
Pediarix (combination of DTaP, Hepatitis B, IPV), PedvaxHIB, Prevnar, and RotaTeq.

You will be given the opportunity to discuss your concerns and ask any questions that you may have.

6 Months

We recommend your child be seen in the office at 6 months of age for a well-child visit.

At this visit you can expect the provider to examine your baby and discuss the following: growth and development, safety, nutrition, and sleep.

We will obtain a naked weight, length, and head circumference.

We will ask if you have city or well water to determine the need for fluoride supplementation. If you have well water, you will be asked to purchase a water test kit to send to the state lab for fluoride testing.

Your child will receive the following vaccines: Pediarix (combination of DTaP, Hepatitis B, IPV), Prevnar, and RotaTeq. Depending on the time of year, we may recommend your child receive his/her first flu shot with a booster dose after 30 days.

Based on the requirements of your insurance company, we may ask you to complete a developmental screening questionnaire.

You will be given the opportunity to discuss your concerns and ask any questions that you may have.

9 Months

We recommend your child be seen in the office at 9 months of age for a well-child visit.

At this visit you can expect the provider to examine your baby and discuss the following: growth and development, safety, nutrition, and sleep.

We will obtain a naked weight, length, and head circumference.

If your child is up to date on vaccines, he/she will not receive any vaccines at this visit; however, your child may need a flu shot depending on the time of year.

Based on the requirements of your insurance company, we may apply fluoride varnish to your child’s teeth (if teeth have erupted).

You will be given the opportunity to discuss your concerns and ask any questions that you may have.

12 Months

We recommend your child be seen in the office at 12 months of age for a well-child visit.

At this visit you can expect the provider to examine your baby and discuss the following: growth and development, safety, nutrition, and sleep.

We will obtain a naked weight, length, and head circumference.

We will obtain a blood sample by finger or toe prick to perform a CBC (Complete Blood Count) to assess for anemia.

We will ask you to answer questions related to lead exposure, and if indicated, we will perform a lead test along with the CBC.

We will also ask you to review our tuberculosis screening questionnaire. If there is a positive risk factor, we may place a PPD (TB skin test) on your child’s arm.

Your child will receive the following vaccines: MMR, Varicella (Chickenpox), Prevnar, and Hepatitis A (and flu shot depending upon the time of year).

Based on the requirements of your insurance company, we may apply fluoride varnish to your child’s teeth (if teeth have erupted), and ask you to complete a developmental questionnaire.

You will be given the opportunity to discuss your concerns and ask any questions that you may have.

15 Months

We recommend your child be seen in the office at 15 months of age for a well-child visit.

At this visit you can expect the provider to examine your baby and discuss the following: growth and development, safety, nutrition, and sleep.

We will obtain a naked weight, length, and head circumference.

Your child will receive the DTaP and PedvaxHib vaccines (and flu shot depending upon the time of year).

Based on the requirements of your insurance company, we may apply fluoride varnish to your child’s teeth.

You will be given the opportunity to discuss your concerns and ask any questions that you may have.

18 Months

We recommend your child be seen in the office at 18 months of age for a well-child visit.

At this visit you can expect the provider to examine your baby and discuss the following: growth and development, safety, nutrition, and sleep.

We will obtain a naked weight, length, and head circumference.

Your child may receive the second Hepatitis A vaccine if it has been at least 6 months since the first one was administered (and flu shot depending upon the time of year).

We will ask you to complete an autism screening questionnaire (“M-Chat”).

Based on the requirements of your insurance company, we may apply fluoride varnish to your child’s teeth and ask you to complete a developmental questionnaire.

You will be given the opportunity to discuss your concerns and ask any questions that you may have.

2 Years

We recommend your child be seen in the office at 2 years of age for a well-child visit.

At this visit you can expect the provider to examine your child and discuss the following: growth and development, safety, nutrition, behavior, potty training, and sleep.

We will obtain a naked weight, length, and head circumference.

We will ask you to complete an autism screening questionnaire (“M-Chat”).

We will ask you to review our cholesterol screening questionnaire. If there is a positive risk factor, we may obtain a blood sample by finger prick to perform a lipid panel.

We will also ask you to review our tuberculosis screening questionnaire. If there is a positive risk factor, we may place a PPD (TB skin test) on your child’s arm.

Your child may receive the Hepatitis A vaccine if not previously given (and flu shot or flu mist depending upon the time of year).

You will be given the opportunity to discuss your concerns and ask any questions that you may have.

2 1/2 year

We recommend your child be seen in the office at 2 ½ years of age for a well-child visit.

At this visit you can expect the provider to examine your child and discuss the following: growth and development, safety, nutrition, behavior, potty training, and sleep.

Your child will be weighed with clothes on and shoes off using the regular stand-up scales.

We will obtain his/her height using the measuring device on the wall. We will also calculate your child’s BMI (Body Mass Index).

Your child may receive the Hepatitis A vaccine if not previously given (and flu shot or flu mist depending upon the time of year).

We will ask you to complete a developmental screening questionnaire.

3 Years

Your child will be weighed with clothes on and shoes off using the regular stand-up scales. We will obtain his/her height using the measuring device on the wall. We will also calculate your child’s BMI (Body Mass Index) and check his/her blood pressure.

We will perform a vision test in the hallway using pictures/shapes.

We will ask you to review our cholesterol screening questionnaire. If there is a positive risk factor, we may obtain a blood sample by finger prick to perform a lipid panel.

We will also ask you to review our tuberculosis screening questionnaire. If there is a positive risk factor, we may place a PPD (TB skin test) on your child’s arm.

Your child will be weighed with clothes on and shoes off using the regular stand-up scales.
We will obtain his/her height using the measuring device on the wall. We will also calculate your child’s BMI (Body Mass Index) and check his/her blood pressure.

We will perform a vision test in the hallway using pictures/shapes.

We will also check your child’s hearing using a machine that produces “beeping” sounds.

We will ask you to complete a developmental screening questionnaire about your child.

We will ask you to review our cholesterol screening questionnaire. If there is a positive risk factor, we may obtain a blood sample by finger prick to perform a lipid panel.

We will also ask you to review our tuberculosis screening questionnaire. If there is a positive risk factor, we may place a PPD (TB skin test) on your child’s arm.

Your child will receive the following vaccines: Kinrix (combination of DTaP and IPV), MMR, Varicella (Chickenpox), and Hepatitis A- if series not previously completed (and flu shot or flu mist depending upon the time of year).

You will be given the opportunity to discuss your concerns and ask any questions that you may have.

5 Years

We recommend your child be seen in the office at 5 years of age for a well-child visit.

At this visit you can expect the provider to examine your child and discuss the following: growth and development, safety, nutrition, behavior, exercise, sleep, and school.

Your child will be weighed with clothes on and shoes off using the regular stand-up scales. We will obtain his/her height using the measuring device on the wall. We will also calculate your child’s BMI (Body Mass Index) and check his/her blood pressure.

We will perform a vision test in the hallway using pictures/shapes.

We will also check your child’s hearing using a machine that produces “beeping” sounds.

We will ask you to complete a developmental screening questionnaire about your child.

We will ask that your child urinate into a cup so we can run a urinalysis test in the office.

We will obtain a CBC (Complete Blood Count) to assess for anemia. A blood sample is collected by pricking your child’s finger and running the test in our in-house lab.

We will ask you to review our cholesterol screening questionnaire. If there is a positive risk factor, we may perform a lipid panel along with the CBC.

We will also ask you to review our tuberculosis screening questionnaire. If there is a positive risk factor, we may place a PPD (TB skin test) on your child’s arm.

If your child did not receive the 4 year vaccines (Kinrix, MMR, and Varicella) he/she will receive those at this visit. Your child may receive the Hepatitis A vaccine if not previously given (and flu shot or flu mist depending upon the time of year).

You will be given the opportunity to discuss your concerns and ask any questions that you may have.

6 Years - 9 Years

We recommend your child have a well-child visit annually.

At these visits you can expect the provider to examine your child and discuss the following: growth and development, safety, nutrition, behavior, exercise, sleep, and school.

Your child will be weighed with clothes on and shoes off using the regular stand-up scales. We will obtain his/her height using the measuring device on the wall. We will also calculate your child’s BMI (Body Mass Index) and check his/her blood pressure.

We will ask you to review our cholesterol screening questionnaire. If there is a positive risk factor, we may obtain a blood sample by finger prick to perform a lipid panel.

We will also ask you to review our tuberculosis screening questionnaire. If there is a positive risk factor, we may place a PPD (TB skin test) on your child’s arm.

Your child may receive the Hepatitis A vaccine if not previously given (and flu shot or flu mist depending upon the time of year).

10 Years

We recommend your child have a well-child visit annually.

At this visit you can expect the provider to examine your child and discuss the following: growth and development, safety, nutrition, behavior, exercise, sleep, school, and high-risk behaviors.

Your child will be weighed with clothes on and shoes off using the regular stand-up scales. We will obtain his/her height using the measuring device on the wall. We will also calculate your child’s BMI (Body Mass Index) and check his/her blood pressure.

We will ask you to review our cholesterol screening questionnaire. If there is a positive risk factor, we may obtain a blood sample by finger prick to perform a lipid panel.

We will also ask you to review our tuberculosis screening questionnaire. If there is a positive risk factor, we may place a PPD (TB skin test) on your child’s arm.

Your child may receive the Hepatitis A vaccine if not previously given (and flu shot or flu mist depending upon the time of year).

Once a female has started her menstrual cycle and has had at least 3 cycles, we will obtain a urine specimen in a cup for urinalysis and a blood sample by finger prick to perform a CBC (Complete Blood Count) to assess for anemia.

Your child may receive the Hepatitis A vaccine if not previously given (and flu shot or flu mist depending upon the time of year).

Based on the requirements of your insurance company, we may check your child’s vision and hearing.

You will be given the opportunity to discuss your concerns and ask any questions that you may have.

11 Years

We recommend your child have a well-child visit annually.

At this visit you can expect the provider to examine your child and discuss the following: growth and development, safety, nutrition, behavior, exercise, sleep, school, and high-risk behaviors.

Your child will be weighed with clothes on and shoes off using the regular stand-up scales. We will obtain his/her height using the measuring device on the wall. We will also calculate your child’s BMI (Body Mass Index) and check his/her blood pressure.

We will ask you to review our cholesterol screening questionnaire. Even if there are no risk factors, we will obtain a blood sample by finger prick to perform a lipid panel as per the AAP guidelines.

We will also ask you to review our tuberculosis screening questionnaire. If there is a positive risk factor, we may place a PPD (TB skin test) on your child’s arm.

Once a female has started her menstrual cycle and has had at least 3 cycles, we will obtain a urine specimen in a cup for urinalysis and a blood sample by finger prick to perform a CBC (Complete Blood Count) to assess for anemia.

It is recommended that your child receive the following vaccines:
Tdap (Boostrix or Adacel) (required by NC law prior to entering the 6th grade), Meningococcal conjugate vaccine (Menveo), HPV (Gardasil), and Hepatitis A vaccine if not previously given (and flu shot or flu mist depending upon the time of year). As of July 1, 2015, the Tdap and Meningococcal conjugate vaccine will be required prior to entering 7th grade or by 12 years of age, whichever comes first.

You will be given the opportunity to discuss your concerns and ask any questions that you may have.

12 Years - 21 Years

We recommend your child have a well-child visit annually.

At these visits you can expect the provider to examine your child and discuss the following: growth and development, safety, nutrition, behavior, exercise, sleep, school, and high-risk behaviors.

Your child will be weighed with clothes on and shoes off using the regular stand-up scales. We will obtain his/her height using the measuring device on the wall. We will also calculate your child’s BMI (Body Mass Index) and check his/her blood pressure.

We will ask you to review our cholesterol screening questionnaire. If there is a positive risk factor, we will obtain a blood sample by finger prick to perform a lipid panel.

We will also ask you to review our tuberculosis screening questionnaire. If there is a positive risk factor, we may place a PPD (TB skin test) on your child’s arm.

Once a female has started her menstrual cycle and has had at least 3 cycles, we will obtain a urine specimen in a cup for urinalysis and a blood sample by finger prick to perform a CBC (Complete Blood Count) to assess for anemia.

The parent may be asked to leave the room during the examination portion of the visit to respect the child’s privacy. The provider may wish to speak with the adolescent alone to do more anticipatory guidance and counsel on high-risk activities.
